Year figures use 13 fills for weekly injections (a box lasts 28 days) and 12 for monthly bottles of tablets. The HSA/FSA column takes off federal income tax at the rate you chose and nothing else; it only applies if the costs qualify, which the IRS limits to treatment of a diagnosed disease such as obesity.
Prices that change on 1 January 2027
- Lilly's savings card terms, the Zepbound and Foundayo refill prices and the Foundayo offer prices end on 31 December 2026.
- Novo Nordisk's $199 starter price for new Wegovy and Ozempic patients ends on 31 December 2026.

What TrumpRx.gov is
Launched on 5 February 2026, it lists out-of-pocket prices for Wegovy, Ozempic, Zepbound vials and Foundayo and passes your order to the maker. On 18 October 2026 its prices matched LillyDirect and NovoCare exactly. If you have insurance, check your copay first.
About compounded semaglutide and tirzepatide
Compounded versions are often cheaper, and the rules around them have tightened. This is where they stand:
- The FDA declared the tirzepatide shortage over on 19 December 2024 and the semaglutide shortage over on 21 February 2025. The grace periods for compounding pharmacies ended in spring 2025.
- On 6 February 2026 the FDA said it would restrict GLP-1 ingredients in non-approved compounded drugs, and on 3 March 2026 it sent warning letters to 30 telehealth companies over how they marketed them.
- Compounded versions are not FDA-approved, may not be called generic or the same as the approved drug, and some use salt forms of semaglutide that are a different active ingredient.
- On 30 April 2026 the FDA proposed removing semaglutide, tirzepatide and liraglutide from the list larger outsourcing facilities can compound from. No final decision had been published by 18 October 2026.
Spending less without changing medicine
- Refill on time. Several Lilly prices are lower only if the next fill comes within 45 days.
- Ask whether your plan covers it for another approved use: heart risk, sleep apnea or fatty liver for Wegovy and Zepbound, type 2 diabetes for Ozempic and Mounjaro.
- If your plan says no, appeal. The appeal letter writer drafts one.
- Use pre-tax money. Prescription drugs count as medical expenses. Weight-loss costs count only when they treat a specific disease diagnosed by a physician, such as obesity, high blood pressure or heart disease, not for general health or appearance. Drugs imported from other countries generally do not count.
