What the labels say, and where the numbers come from
Labels for weight-management medicines no longer print a BMI number. They say the medicine is for adults with obesity, or with overweight and at least one weight-related condition. The figures everyone quotes, 30, or 27 with a condition, are who the clinical trials enrolled. Insurers and Medicare reuse them, sometimes with higher bars, which is why a plan can say no to someone a prescriber would treat.

Questions people ask
What BMI do you need for a GLP-1?
The current FDA labels for Wegovy, Zepbound and Foundayo say adults with obesity, or overweight with at least one weight-related condition. The trials behind them enrolled people with a BMI of 30 or more, or 27 or more with a condition such as high blood pressure, high cholesterol, type 2 diabetes, sleep apnea or heart disease, and most insurers use those numbers.
Can I get a GLP-1 with a BMI under 27?
Not for weight management under the label. If you have type 2 diabetes, medicines such as Ozempic and Mounjaro are approved for blood sugar control at any weight, and that decision is your prescriber's.
Does Medicare cover weight-loss GLP-1s now?
Through the Medicare GLP-1 Bridge, from 1 July 2026 to 31 December 2027, eligible Part D enrollees pay $50 a fill for Wegovy (injection or tablets), Foundayo, and the Zepbound KwikPen only (not Zepbound single-dose pens or vials). The BMI rules are stricter than the labels: 35 or more, or lower with specific heart, blood pressure, kidney or blood sugar conditions.
