Another GLP-1 pill reaches 12% weight loss in a mid-stage trial

In a phase 2b trial published in Nature Medicine, the oral small-molecule GLP-1 aleniglipron produced 9.0%, 10.7% and 12.1% weight loss at three doses over 36 weeks, against 0.5% on placebo.

Two GLP-1 pills reached the market inside four months. A third is now through mid-stage testing, and the results were published in August 2026.

What happened

A phase 2b randomised, double-blind, placebo-controlled trial of aleniglipron, an oral small-molecule GLP-1 from Structure Therapeutics, was published in Nature Medicine on 10 August 2026. It enrolled 230 adults with an average age of 50 across 38 medical centres in the United States, with doses escalated every four weeks.

Over 36 weeks, weight fell by 9.0% on 45 mg, 10.7% on 90 mg and 12.1% on 120 mg, against 0.5% on placebo. Gastrointestinal side effects were described as generally mild to moderate and became less frequent over time. Around one participant in ten, 10.4%, stopped treatment. No drug-induced liver injury was reported, which matters because liver signals have ended earlier small-molecule programmes in this class.

Robert Kushner of Northwestern Medicine, who worked on the trial, is a familiar name in obesity medicine, and phase 2b is the stage at which a drug earns the right to a phase 3 programme rather than a licence.

What a crowded pill field actually changes

Nothing about your prescription this year. Aleniglipron is not approved anywhere, and a mid-stage result is a long way from a pharmacy.

What it changes is the direction of the next few years. Injectable GLP-1s are peptides, which are expensive to make and need cold storage and a needle. Small molecules are made like ordinary tablets, shipped like ordinary tablets, and can be manufactured at scale by more than one company. Three credible oral programmes reaching the same 11 to 12% range means competition on price and on access, in a category where both have been the real barrier rather than the science.

It is also worth noticing what the number is not. Twelve per cent over 36 weeks is a serious result and it is not 20%. The pattern across the class is holding: the convenient options are landing meaningfully below the strongest injections, and anyone choosing between them is trading effect for ease rather than getting both.

What it means for you

Nothing to act on today. If you are in a conversation about starting, or about switching because of cost or supply, it is reasonable to ask your prescriber what is expected to reach the market in the next two years, because the answer is no longer "another injection".
