The GLP-1 Diet Advice Gap: Trials Almost Never Measured What People Ate

A scoping review of 129 GLP-1 trials found only 10 reported dietary outcomes. Eating guidance on a GLP-1 comes from nutrition science, not from the drug labels.

Search "GLP-1 diet" and you will find a thousand food lists. Almost none of them trace back to the trials that got these medications approved. A special report by Venya Patel, published by Nutrition Insight on 9 July 2026, explains why: the clinical trials behind liraglutide, semaglutide and tirzepatide almost never recorded what participants actually ate.

What happened

The report is built on a scoping review in Advances in Nutrition covering 129 randomised controlled trials. Of those, 57 described lifestyle modification as part of the intervention. Only 36 recorded diet quality and food intake. Just 10 reported dietary outcomes. Seventeen assessed food cravings or eating behaviour, and half of the dietary assessments that did happen were single time-point measures, such as one ad libitum meal or a buffet. One meal, once, standing in for months of eating.

For accuracy: the underlying review was published in 2025. The July 2026 Nutrition Insight analysis is what put it in front of the nutrition industry.

A booming market on a thin base

That matters because a companion nutrition market has grown up around these drugs at speed. Ingredient suppliers interviewed for the report, including FrieslandCampina Ingredients, Probi, Roquette and HTBA, made two points worth holding onto. Reduced appetite and slower gastric emptying push fibre and micronutrient intake down, not just calories. And lean mass was rarely measured in the original approval studies at all.

So the question of what to eat on a GLP-1 was never a trial endpoint. The answer has to come from nutrition science instead.

What it means for you

Here is the reframe that follows. The medication already did the restricting. Your appetite is smaller, sometimes dramatically so. The risk on this treatment is not overeating, it is under-eating the things that protect you.

A quarter to a third of weight lost on a GLP-1 can be lean mass without deliberate effort. So an eating plan on this medication is a protection plan: enough protein spread across the day, fibre raised gently so digestion keeps moving, water, and resistance training to give the protein somewhere to go. Not a shorter list of foods. A denser one. Our full GLP-1 eating guide walks through it.

Small appetite, high standards for every bite.

What to do with this

That gap is exactly why this site exists. The trials measured the drug; nobody measured the plate. The calculator at least makes the plate explicit: a target, a protein floor, and an estimate of how much you are likely to manage at your dose.

People also ask
Did the GLP-1 trials measure what people actually ate?
Almost never. A scoping review in Advances in Nutrition covered 129 randomised controlled trials. Of those, 57 described lifestyle modification as part of the intervention, only 36 recorded diet quality and food intake, and just 10 reported dietary outcomes. Seventeen assessed food cravings or eating behaviour, and half of the dietary assessments that did happen were single time point measures, such as one ad libitum meal or a buffet. One meal, once, standing in for months of eating.
If the trials did not measure diet, where does GLP-1 eating advice come from?
Nutrition science, not the drug labels. What to eat on a GLP-1 was never a trial endpoint, so the thousand food lists you find when you search for a GLP-1 diet do not trace back to the studies that got these medicines approved. Lean mass was rarely measured in the original approval studies either.
What does that mean for how I should eat on a GLP-1?
It flips the usual framing. The medication already did the restricting, and your appetite is smaller, sometimes dramatically so. The risk on this treatment is not overeating, it is under eating the things that protect you. Reduced appetite and slower gastric emptying push fibre and micronutrient intake down, not just calories.
