GLP-1s and menopause: why the drug still works, and what to protect

Menopause changes where you gain weight and how fast you lose muscle. New analyses show GLP-1s work just as well after menopause, if you protect muscle and bone. Here is how.

The 30-second summary

  • GLP-1s work just as well after menopause. A 2026 analysis of the SURMOUNT program found tirzepatide delivered similar weight loss across all reproductive stages.
  • Menopause already speeds muscle and bone loss as oestrogen falls. A GLP-1's rapid loss on top is a second subtraction, so protection matters more, not less.
  • The tools are unglamorous and proven: enough protein, resistance training, and a conversation about hormone therapy with your prescriber.

The good news first

Many women reach for a GLP-1 precisely because menopause changed everything: the weight settled around the middle, the old routines stopped working, and the scale would not move. It was a fair worry whether the drug would even work against that hormonal headwind.

It does. A 2026 analysis splitting the large SURMOUNT tirzepatide trial by reproductive stage found weight loss of roughly 20 percent whether women were premenopausal, perimenopausal, or postmenopausal. The medication does not weaken in midlife.

The asterisk that matters

What changes in menopause is not the drug's power, it is the stakes around muscle and bone.

From the early forties, falling oestrogen speeds the natural loss of muscle and thins bone by around one percent a year on its own. Layer a GLP-1's fast weight loss on top and you have two forces pulling the same direction. This is why the muscle and bone question, which matters for every woman on a GLP-1, matters most here. (See GLP-1s in perimenopause and bone density on a GLP-1.)

The three levers

  • Protein. A floor of 1.2 to 1.6 grams per kilogram of goal weight, about 120 grams a day, gives muscle the raw material to hold on. (See protein on a GLP-1.)
  • Resistance training. Two short sessions a week loads muscle and bone at once, the single best insurance against losing the wrong kind of weight. (See strength training that fits a tired body.)
  • Hormone therapy, considered. HRT treats menopause symptoms in their own right and, in early research, may modestly add to GLP-1 weight loss. It is your prescriber's call, made with your history.

If you have seen low-dose GLP-1 medicines marketed for midlife, read microdosing in perimenopause and menopause for what the evidence does and does not cover.

What to do with this

Menopause is already working against muscle and bone, and a GLP-1 accelerates the loss unless protein is deliberate. The calculator sets the floor from adjusted body weight rather than scale weight, which is the difference between a number that protects you and one that flatters you.

People also ask
Do GLP-1s work after menopause?
Yes. A 2026 analysis of the SURMOUNT trial found tirzepatide produced about the same weight loss, roughly 20 percent, whether a woman was pre, peri, or postmenopausal. The drug does not lose its power after menopause, though protecting muscle and bone matters more.
Why is it harder to lose weight during menopause?
Falling oestrogen shifts fat toward the middle, slows muscle maintenance, and can worsen insulin resistance, so the same habits stop working. A GLP-1 addresses appetite and insulin sensitivity, which is why it still helps, but the muscle and bone question becomes sharper.
Can I take a GLP-1 with HRT?
Many women do, and early research suggests hormone therapy may modestly add to the weight loss. HRT and GLP-1s are decided by your prescriber based on your history, so raise both together at your appointment rather than choosing between them alone.
